History: Age 32. 2 yrs ago, period with severe burning at micturition and ejaculation, subsided after a course of antibiotics. Intermittent episodes of slight pain in the inguinal area, bilaterally. 2 weeks preceding current examination, recurrence of pain in both groins radiating to the testicles, combined with low ejaculatory flow and volume, crampy pain following ejaculation, lasting for ½-1 hr. Drach-class: NBP. TRUS: Moderately dilated ejaculatory ducts, no obvious cause of obstruction. Edema? Slighty dilated seminal vesicles. Spermiogram: Count at lower end of normal range (32 M/ml), severe asthenospermia (the patient has a 5 yrs old child). Therapy: Initial therapy: conservative measures combined with antibiotics (lomefloxacin) and antiinflammatory drugs (ketoprofene).
